Health Needs Assessment

We are taking part in the regional Better Health at Work Award scheme aimed at improving the health, safety and wellbeing of the workforce. Part of this work will involve looking at health training, work based health interventions, policies and our culture around wellbeing at work. It will also include displays, posters and other campaigns and events taking place. This scheme is coordinated by the Northern TUC and more details may be found at www.betterhealthatworkaward.org.uk

By completing the following questionnaire, you will help us and future Health Advocates to address the health issues that are important to you. The questionnaire should take no more than 5 minutes to complete.

This Health Needs Assessment is entirely confidential and we encourage you to be as honest and open as you can be. You will not be asked to enter your name or any personal identifiable information and none of the questions are compulsory.
